Every two years, the County of San Diego's Community Action Partnership (CAP) develops a Community Action Plan.
The Plan aims to improve CAP's services and address the priority needs of San Diego’s low-income community members. The results from a Community Needs Assessment inform the Plan.
The Community Needs Assessment involves the following:
a survey to gather anonymous feedback from community members
in-person meetings where community members can share their experiences and ideas for solutions
a report to share the data gathered from the survey and in-person meetings
Through the anonymous responses, the Survey will help CAP understand:
the conditions that affect low-income community members
the resources needed to address the unmet needs of the most vulnerable residents
what current services are helpful and what need improvement
the barriers to services and any gaps in services
ideas for future programs and services
How to Get Involved
Complete the survey through July 31st
Join an in-person meeting through August 25th
Ask questions

The Assessment results will guide the Plan, which will direct the resources where low-income residents need them most.
While the County uses the Plan to guide CAP programs, the data in the Plan and report may be helpful for others working in low-income communities.

Phase 1: Initial Community Engagement (Summer 2026)

Survey period (July 9, 2026 - July 31, 2026)
Join an in-person Ideas Exchange meeting nearby (July 31, 2026 - August 25, 2026)
Phase 2: Draft Plan and Report (Fall - Winter 2026/2027)
this is an upcoming stage for Community Action Plan
The Draft Community Action Plan and countywide Community Needs Assessment Report will be published for public review and comment. (January - February 2027)
A Public Hearing will be held to discuss the draft plan and report. (February 2027)
Phase 3: Final Plan and Report (Spring 2027)
this is an upcoming stage for Community Action Plan
The Final Community Action Plan with Community Needs Assessment Report will be published. (June 2027)
Follow-up presentations to share insights forto each of the HHSA communities will be held. (May - June 2027)
